Alchemist Bug (Dev form not working for me, someone else please submit)

I tried sending to the Dev team but for some reason the form isn't working so hopefully they will see this or someone else can send it.

I just played a game as Alchemist, had salve, tangos, and stout shield. About five minutes into the game my Q and W had zero cooldown and zero mana cost. I could spam the poison and stun until my computer couldn't handle it any longer. The ultimate was fine at first, but then about 20 minutes into the game it too was doing the same thing as the Q and W. I could run around carpet bombing the entire forest on both sides with no one to stop me. So long as I didn't look at the poison when I stacked it I wouldn't lag.

I asked the other team to stop fighting so we could end the game, but they refused, so I abused the glitch and used the match as a way to try to figure out why it was happening. Tried switching out my items, but it changed nothing so I do not think it was item related. The ult turned into the zero cd zero manacost after I spammed R even though it said there was 45sec cooldown.

I posted this so someone else can report it for me since the bug report form is not working on my end. It was definitely fun playing wtf mode for awhile, but this will break the game if it happens too often.
