Ripped skin tattoo advice needed!

I'm planning on getting ripped skin tattoos on each of my calves, with a Irish flag underneath the skin on the right and Italian flag on the left. Anyone with ripped skin tattoos have advice for the design? Should I get some blood draw in to make it more realistic? Should the flags be rippled or taunt? Etc...
