How to replace bathroom vent

The bathroom vent that is supposed to help keep mositure down in my bathroom does not work at all. It comes on but does nothing. I'm not sure if it goes through the roof or what. Images can be viewed here . I don't know anything about how it works or how to replace but reconditions welcome!
