Yeah it seems like if you try to enforce it with evidence that can results in delays and increased gestation past the legal point, and that hurts the rape victim if you believe that abortion access is needed for them. So a lot of countries just end up taking a womens word for it. And because people can lie and word gets around that you can drop the word rape or incest and gain access to abortion, countries that ban abortion but allow it for rape or incest either are functionally abortion on request countries or are heading that direction.

Side discussion, why is incest grounds for abortion? Yeah there is a chance for issues but its just a chance and isnt that discrimination against disabled people? As it says rape and incest are commonly bundled together, why is that?
