Repeated Microscopic Surgery

Hello Guys ,, iam 29 Years old and underwent bilateral microscopic surgery 6 month ago ,, unfortunately till now i have a reflux on lt side with a minimal improvement on semen analysis What You Will Recommend for the next step ?? Should i repeat the surgery or its better to go for Embolization
