Where can I find an example of The Dignan Ego Identity Scale?

Hey there! I am reading an article for a course that mentions The Dignan Ego Identity Scale on multiple occasions. I have tried to search for this to see what it is exactly but have had no luck.

The best understanding that I have found is that "the scale consists of 50 items considered indicative of aspects of ego identity." I so badly want to know what these 50 items are!

Any help would be appreciated it. Thanks so much.
