What was the book/movie where the guy gets killed by the slide coming off his semi-automatic pistol?

Maybe it was a Stephen King book? Not sure. Basically, the bad guy tries to shoot, but the slide flies off, embeds itself in his head, killing him.

Part 2 of the question: is this an actual risk with semi-auto pistols? Can/does this happen?
