What determines where you spawn in multiplayer?

I was just wondering what determines where you spawn in GTAIV multiplayer. I noticed sometimes i will spawn really far from everyone even though there are no spawn settings for the match. Can anyone give me some insight. Is it location, internet connection, or a combination of things.
