A little help from the gunsmiths out there? Hammer is off center on my S&W 6904

So the other day I was looking at my Smith & Wesson 6904 and I noticed the hammer is leaning off to the right, enough so that there is actually wear on the hammer and the slide from use while in this condition.

The gun still functions, but I dislike the wear this is putting on it.

Any advice? Is this something I could probably fix on my own or should I really just take it to a gun smith?
