Why do I get disconnected while playing GTA IV?

So while playing GTA IV I often get "Connection to one or more players has been lost". I also sometimes get a message saying that my router/NAT is too strict but that is my problem, I know. So my question is, how can I get rid of the "Connection to one or more players has been lost problem"?
