What is the worst food accident that you've ever prepared?

For me it was a pumpkin pie when I was about 12. My nephew (1-2 years older than me and my brother, also older) decided we wanted to make a pumpkin pie. We came up with this idea because we was the only ones home, our parents were out of state for the week and we found several large cans of pumpkin in the pantry. We had two problems though. The first is that we had no pie crust or a pie tin. The second, we didn't have any pumpkin pie spice but we didn't know we needed that.

So we got a fairly large pan and coated it with a flour corn meal mixture we thought would be fine to work as crust. Next we dumped in several large cans on pumpkin on top of it. We tossed it in the oven, no preheat or anything for about an hour. We took it out, the outside looked burned and on the inside it looked very runny and gooey. Not only that but our pie crust pretty much turned into corn bread and it was horrible. The whole thing was absolutely disgusting.
