Monitor "splitting"? Image attached to explain.

It's almost as though the image is being "dragged" to the right, and then coming back from the other side.

Also, the monitor is starting to just go black and the computer is locking up at times, but it usually starts with the splitting.

Started happening this morning while using photoshop, but now it's becoming more constant.

Any help would be appreciated!

Edit: I should mention I already tried changing monitors, and cables.
