Beautiful. I don't think I've seen a poem formatted like this before, but that doesn't detract from the message, which is infinitely more important. You've done a wonderful job highlighting dualities in language, showing how it can be beautiful and misleading at the same time. Love it!

I really like the innovation here. I think the strength of the content supports the new, solid/consistent style/format. I like the period at the end of the title.

The title is fine. It might also work with a less on-the-nose title.

The "life is hard" line duo may not be the strongest to end with. The idea of forgetting might even be stronger/more tragic. If you switched the last two couplets and then simplified the last part of the "I don't believe we've met before" couplet to something like "A friend of an old man who's forgotten" or even "A man who's forgotten"---this might be something worth considering.

Very cool and inspiring format. Concise, easily understood and made me ruminate on my life choices. Very effective, thank you for sharing.

The duality is really inspiring. I really liked the last line "man on the ledge". Your language shows the different perspective people have, and the spectrum of feelings. 
I think during the instant when each of the dictotonomous events happen, the respective person feels the exact same thing.  When the kid realized that life is hard, he is feeling the exact same feeling as the man on the ledge, although he might choose a different path.

This is a very unique and beautiful idea! Each couplet showed an original idea, each one nearly bringing me to tears. The last couplet showed a finality that worked to close a poem of this style, and the contrast between each verse was clear. I love this.
