Need some advise for my transgender boyfriend and I.

I wasn't sure where to post this, but it seems appropriate. My partner and I are both 16. He is transitioning from a girl to a guy, and I fully support him and love him and want to continue our relationship, regardless of gender. I'm just not sure how to tell my parents. They are very pro lgbt I know they will be supportive, my anxiety disorder just kills any attempt to talk to them. My partner's family is also pro lgbt. The only problem is that he's just scared his conservative grandparents won't understand. I'm also scared mine won't understand either. I also need some general advise for us as a couple. Tl; Dr: My transgender partner and I need advise, mostly concerning dealing with family, but in general as well.
