Army program grad looking to transition to civilian medical field.

I have spent the last 3 years bouncing back and forth between Active Duty Army and a civilian job. The last year on Active Duty I was in training to become a Respiratory Therapist, which included a civilian degree and certifications. For the moment I have returned to civilian life and am trying to find an entry-level job in in the medical field as a Therapist. I am looking for critiques of my resume as I used the FAQ template to put something together.

A couple of issues I’m wondering about are the overlapping blocks of time in which I have been in the Army and also a package handler. Should I split up the time frames to only include one job for each period of work? I will remain the Army Reserve so the employment is ongoing. Should I limit the time listed to the Active Duty dates? Also, I am only going back about 6 years as the jobs I had before this period were in no way related to the medical field. Would this be a problem to HR? Finally, would a branding statement be appropriate for this resume as its for entering a new field of work? Thank you for your help!
