Dr.Mundo Axes - Not hitting targets?

I wish I had a video of this, but I just haven't been able to get recording software yet. I've had it, multiple times, where I've thrown a briefcase (axe) and it has just gone straight through an enemy champion. At first I thought it was just me missing, but looking at LOLReplay shows that it goes RIGHT THROUGH the enemy without connecting. Also, without recapping the last match I was in, I think Nidalee's spear went through one of the rude dudes.

Has anyone else seen this?
