My cousin refuses to do anything that I want to do, and I don't know what to do. AITJ

I have a cousin who is my best friend. He moved away about a year ago to about an hour drive away. Whenever we have a rare chance to hang out, I think we should work together to do something fun. However, he only accepts the stuff he wants to do. His excuse is always: I get a headache doing whatever I want to do. For example, today he was with me at my house, so I suggested we go swimming or do an escape room. He said that a ton of people being in the same area as he makes him dizzy or something. Now to be "fair" he was in the ER 3 months ago, but that was a while ago. I have seen him be perfectly fine doing crowded things that he wants to do, but I cannot do anything with him apparently. What do I do?
