I think he was added after patch 1.03 or 1.04 when they added more NPC summons to the game. you can also summon Nephilli for this fight

Can’t you not reach the Three Fingers once you kill Maliketh? So you’d have to do it before then.
You can reach them, but I don't know if it would work to summon him
You only need to have done Morgott. You can get there even before going to the lift, in which case Yura's set will be next to the ruins instead of Shabriri with no consequences.
That’s what I did my first run because the Three Fingers sounded like a real stand-up fellow, I only met Shabriri on my second playthrough.

But I believe you can, either just hit the bonfire and do it later, or (maybe with cat ring equivalent) drop down from where the Crimson Amber Medallion+2 is.
There’s an open wing to the sewer in capital of ash that lets you get access to the shunning grounds.
You can still reach the door but can’t go through it to progress that ending
