I spent some time fiddling with the ol' wobble analysis tool today. Periodicity is actually useful now. You can zoom in on individual nights. I got rid of the flow limitation logic as it's outside the scope of this project. The old version is still available for those who prefer it.
It's now more explicitly targeted as a high loop gain detector. There's plenty of other great tools for flow limitation.
Let me know what you think!
ASV is a strange algorithm compared to other PAP modalities. At least on the Resmed side, it's targeting instability in minute vent, not flow limitation or obstruction. It does this by changing inhale pressure for each breath. People who respond to it are likely to have a combination of central apneas and waxing and waning patterns over 30-90 seconds. This instability is nearly entirely independent of anything obstructive. It's a control problem, not a plumbing issue. Though it can absolutely be combined with plumbing issues because nothing in SDB treatment can actually be simple. Flow limitation becomes a constraint rather than the main target.
This makes titration logic vastly different
It's my main issue with UARS as sort of the substrate. I've dealt with lots of the structural stuff (jaw expansion as a kid, septoplasty, turbinate resection, tongue tie release) yet I continue to put my ASV through its paces every night.
I've been literally afraid of sleep since early childhood due to an actually perfectly reasonable fear that I'd forget to breathe as I fell asleep. There were the mild chest pains at sleep onset; those were fun. Any time I mentioned my sleep issues to doctors it was always "more yoga, warm milk" and when those things inevitably didn't work it was clearly because I wasn't trying hard enough. I have counted so many breaths and played so many complicate word
For me it's the fact that anything under my nose makes my cartilage hurt within about 4 hours. I also can't do nasal dilators, clip on earrings, or foam headphones that put pressure on the external ear. My cartilage is just silly.
I've only tried the N20 as far as over the nose nasal only but the diffuser was incredibly annoying.
So yeah, F20 life.
I have always had difficulty getting out of bed since I was a small child. Every morning was a battle. I tried all sorts of hacks and yet nothing ever changed. I could surf a 3 minute snooze interval for 90-120 minutes with light blasting me in the face and doing math problems to earn another snooze and wouldn't even remember the first 45 minutes of the process. These methods kept me from oversleeping but I was miserable. This would happen even if I got to bed at a perfectly reasonable hour and had a full night's sleep.
It turns out that I have upper airway resistance syndrome (UARS) due to a combination of airway anatomy and how easily my brain wakes up with any level of breathing effort.
I started micronized P 5 days ago without estrogen as my provider told me smugly that 90% of people in early peri only need P, and it destroyed my sleep and annihilated my emotional regulation within 3 days. Back in the before times, when I had actual endogenous hormones to work with, I dealt with PMDD. I had a similar intense rage reaction to the mini pill when I tried it way back when.
Has anyone else had this experience? Has being on E at the same time helped to moderate it? Or has an alternative form/delivery for progesterone worked better for you?
Dr. Jon Slotkin on X: "I have a guest essay in @nytimes today about autonomous vehicle safety. I wrote it because I’m tired of seeing children die. Done right, we can eliminate car crashes as a leading cause of death in the United States @Waymo recently released data covering nearly 100 million / X
x.com/slotkinjr/status/1995864439405375903After much swearing and figuring out how to get my first actual website ever hosted and then not blocked by Reddit, here is the newest version of my fancy loop gain/apparent actual sleep badness tool.
As teased in the last post, this version includes an estimated arousal index (how many big weird breaths with surrounding gaps per hour). This is combined with the existing periodicity, regularity, and strict flow limitation scores. The flow limitation detection is far more sensitive than what one gets from OSCAR. Higher scores are worse for all metrics, though I'm still learning what the scores actually mean aside from looking at my own data across the last
I have been watching the thread with the original very rough version and took user input. Minimum size is user defined by duration rather than file size, statistics are time weighted so a terrible short nap doesn't get undue power but also isn't entirely excluded, single night processing is available, and you can gradually add more sessions if you want to compare particular moments in your therapy rather than all or nothing batch or single. I've noticed that flow limitation works a lot better now and clearly shows when I've had colds or have been recovering from nasal surgery.
The readability has also b
