Custom Orthotics - Heel issues is making me hate them!

After complaining of pain in my big toe, the podiatrist diagnosed me with something based on an x-ray (functional something or other) and recommended custom orthotics. I have a pretty big sneaker collection and questioned whether or not it would affect the fit of my shoes, which the doctor assured me they would not. The orthotics came from ComfortFit Labs in New Jersey.

Anyway, I wore them for about a week and kept going nuts because I felt like my heels were moving too much and that my feet were not "secure" within my shoes. I actually wondered if my feet were shrinking until I realized the problem was with the orthotics. It was almost like my feet weren't deep enough inside the shoes in comparison to my factory inserts. No matter how tight I tied my shoes, it didn't help. It wasn't until I did a comparison by leaving a factory insert in one shoe and an orthotic in the other did I realize that the orthotic was causing the heel slippage (no, it doesn't pop out - just increased movement).

I returned to the doctor and he sent them back to remove the "post" from the heel, which he said would lower the orthotic and should eliminate my issues. If it did lower it, it sure doesn't feel like it, because my feet, particularly my heel and back part of my foot, still do not feel secure. I cannot imagine how anyone would be able to do any intense physical activity in these things without going nuts.

I sometimes wonder if the issue isn't the heel, but the arch support being overkill and is causing my feet to not "seat" properly in my shoes. Yes, I am aware of the break-in process and having to adjust - but I'm pretty sure that "adjusting" to heel slip isn't part of break in, is it? The only other issue I experience is that sometime feel pain around the tarsals, which I attribute to the extreme arch support causing my feet to press really hard against the top of my shoes.

Very upsetting, because despite my complaints, the orthotics do help with the toe pain. I don't go back until Sep 15th, but am worried that I wasted 600 dollars and he's just going to say there's nothing else they can do.

Has anyone else experienced these issues? Is needing adjustments (twice) normal practice?
