Health insurance denying physical therapy

My plan is supposed to cover 120 sessions per year, but after 12, they say it’s not medically necessary and deny 80% of the pre-approved visits requested by my physical therapist.

Technically I only have patella maltracking, but my TT-TG distance is >20mm (it’s 23 mm). I had a tibia tubercle osteotomy on my left knee after a very severe dislocation event where my patella stayed dislocated and on the side of my leg for 6 weeks until I could her into surgery for TTO and MPFL reconstruction and meniscus repair.

Well after being Non weight bearing on my left leg for 3 months (pre and post op combined), I now have pain and loud crunching in my right knee. Was diagnosed with maltracking due to the deformity with my tibia tubercle.

I’m going to need TTO surgery on my right leg if I cannot do the physical therapy. But my insurance doesn’t give a shit about me and the fact that I can’t walk down stairs as a 26 year old woman. This has absolutely ruined my career as an environmental engineer. I’m not even physically well enough in my left leg to even consider surgery in my right leg for at least 1-2 more years. I need PT!!!

What have yall done to get insurance to cover PT? I’m losing my mind here. I work so hard, but as soon as the PT starts to help, they cut me off. And my mobility is still pretty crappy and anything I do at my own gym just causes me a ton of pain. I really need to be with my PT.

TIA
