Can you grow new fat cells as an adult? Yes, and the skin is unusually good at it.

The common intuition is that you're stuck with the fat cells you were born with. The published evidence says something more interesting.

What the research shows:

• Adipocyte renewal continues throughout adult life. Spalding et al. 2008 used atmospheric radiocarbon dating to retrospectively birth-date individual adipocytes and estimated approximately 8 to 10 percent annual renewal of adipocytes in subcutaneous abdominal fat, regardless of age or BMI (PMID 18454136).

• Renewal capacity varies by anatomical location. In human in vitro work, primary facial preadipocytes show greater adipogenic differentiation capacity than abdominal subcutaneous preadipocytes (Chon and Pappas 2015 in Experimental Dermatology, PMID 26167398). Translation: the precursors that become fat cells in your face are intrinsically more "willing" to differentiate than the precursors in your belly.

• The adipogenesis process itself is multi-step: stem cell commitment, preadipocyte proliferation, and PPARγ-driven differentiation into mature adipocytes (Rosen and Spiegelman 2014, PMID 24439368). Each step is biologically modifiable. TNF-α inhibits PPARγ activity and suppresses adipogenic differentiation (Cawthorn and Sethi 2008, PMID 18037376).

The caveat: direct in vivo measurement of how fast human dermal or facial adipocytes are replaced specifically remains unavailable. The intrinsic differentiation capacity is well documented; the kinetics in living human dermis are still inferred rather than directly measured.

What this means for the "fat cells are permanent" intuition: it's wrong, but the timescale is years, not weeks. Adipogenesis is a slow background process that responds to the local inflammatory environment, not just age.
