HSE line manager sent incorrect reference for internal job please help

I was put on a panel for hse clerical position , I previously worked in the hse as an agency worker and was let go due to embargo . I was offered a position in the same department I used to work in from the hse panel , I sent the hse standard template reference form to my former line manager multiple times along with numerous phone calls to request the form to be sent to the recruitment department. He did not respond to any requests . The recruitment dept said that it was their policy to have a hse reference if you previously worked there , I explained the difficulty I was having with getting him to fill the hse reference form , I contacted the hr department in the hse and they told him to fill out the forms and send to recruitment . In the end all he did was send 2 line email verifying I had worked there , I got a call from recruitment to tell me that not only did I not get the job but I was being taken off the panel as well and was told that I would never get a job in the hse because he didn’t fill the correct reference hse form , I am so upset over all of this as I was a great worker there . I was told when he was asked to do the form his answer was he didn’t do references but this was for the job I previously worked in and he was the only one I could get the reference from , any advice would be appreciated
