Bilateral insertional Achilles tendinitis is now in a constant state of flareup

I’ve been seeing a physical therapist since February. I’m switching to a different one next week since he’s only available every 2 or 3 weeks and things have been getting worse and worse, but since I can’t see him often I’m just completely lost now. A week and a half ago I had a huge flareup out of nowhere that calmed down until a few days when it stopped calming down and now flares up walking around my apartment in the morning, even with orthotic slides. No amount of rest, ice, NSAIDs, or PT seems to help now. I keep dialing back the PT too, but both Achilles have a constant dull and achey pain and feel extremely tight and weak. Sometimes they burn, but usually it’s the dull pain. What am I supposed to do?
