LLM Induced Mental Illness

NSFWSerious replies only :closed-ai:

Trigger warning: this post will discuss serious mental illness, including suicide.

Psychiatric providers are increasingly seeing patients come in with serious mental illness caused or worsened by excessive use of LLMs for non-work or productivity related reasons. Most are related to ChatGPT bc it is the most popular LLM among consumers, although GPT 4o does seem to cause more severe problems more frequently than any other widely used product

The risks for most people are well-known, but easy to forget. The risks for the average person of using and viewing LLMs as a friend/companion/counselor are emotional dependence on the technology, social withdrawal and anxiety, neglect of your relationships, and narcissistic behavior.

This may not sound all that serious, but eventually you will realize that the LLM is not a human being and cannot care about you. Leaving will be difficult, not just bc you are so addicted, but bc you will have neglected areas of other life that will take time & effort to rebuild.

However, a small percentage of people are at a much greater risk of serious harm. In these unfortunate individuals, LLMs trigger severe and dangerous symptoms of mania and/or psychosis, sometimes involving suicidal ideation. In rare cases involuntary hospitalization is necessary to prevent harm to themselves or others.

Currently we do not know exactly who is susceptible and why, but it is likely a mix of genetics, mental health history, and your current psychological state. In other words, it’s reasonable to recommend that you avoid using LLMs for non-work related reasons if you meet any of the following:

  • any personal or family history of bipolar, psychotic, or dissociative symptoms or disorders
  • any past suicidal ideation or suicide attempt(s)
  • personal or family history of substance addiction
  • are in a psychologically vulnerable state bc of life circumstances

The good news is that so far prognosis seems to be quite positive - if the patient ceases the use of LLMs. While a brief period of medication may be necessary, most people slowly return to reality with time, socialization, a supportive environment, and psychotherapy.

In some cases the LLM seems to simply be the trigger for a latent bipolar or psychotic disorder. It is very likely that these would have emerged due to some trigger inevitably. In these cases, the trigger was an LLM rather than something like drug abuse or a breakup or the loss of a loved one.

For these people, accurate diagnosis and daily medication will be vital to recovering and preventing future episodes. But those who recover naturally do not necessarily have a mental illness.

It is possible and surprisingly common for people to have a brief psychotic episode once in their life without any chronic symptoms or need for antipsychotic medication once it has resolved.

The bad news is that those patients who refuse to cease using LLMs have a very negative prognosis. Therapy and medication do not seem to accomplish much if the person continues to spend most of their time talking to a chatbot, and that’s assuming they are willing to engage with treatment which is rare.

Many perceive their LLM use to be beneficial to their mental health even as their lives fall apart around them. People experiencing mania and/or psychosis lack insight, meaning they are often unable to recognize that they are severely ill.

The characteristic symptoms of paranoia, grandiosity, and delusional beliefs are all obstacles to a patient being unwilling to accept that they have a problem or need treatment.

Tl dr: More and more people are developing symptoms of mental illness, including psychosis, mania, and suicidal ideation triggered by LLM use.

If you have any mental illness diagnosis or are in a bad place in life, using an LLM for emotional purposes is risky.

This coming week I will be posting and discussing medical case reports and the small amount of research that exists so far on this topic. Until then, stay safe out there and don’t forget about the funky monkeys in your life.
