Renewing Passport - Want the Book but Only Have the Card Without Sleeve

I am trying to apply for a passport book, but only have my passport card that expired earlier this year. I am also missing the sleeve it came with.

Even with the sleeve missing, can I still check the box for: "The U.S. passport book and/or card that I am renewing has not been mutilated, damaged, or reported lost or stolen." or should I start a completely new application instead of renewal?
