IVF with UHC Insurance (USA)

Curious if anyone has been able to get IVF covered under United Healthcare. Currently they define infertility as unable to conceive after trying for 12 months minimum but can having CMT be a sufficient reason to have it covered. Also if you were able to get insurance to cover, what did your doctor file with your insurance as the reason?
