How to handle permanent transformations into a creature???

So in one of the campaigns I'm playing in I've acquired a Mothman Memento and it says that "A creature that dies from the nightmare effect rises as a mothman the next morning."

Assuming I died to the nightmare effect and "rose as a mothman", what stats would I inherit/what wouldn't I? Would I be able to use the spell-like abilities of a mothman? What about skills, feats, other abilities, etc? I'm not quite sure where to start with this. I know I'd probably inherit physical ability stats at the very least, but other than that it's pretty murky territory.

I'm pretty sure this would usually entail losing control of your character. You'd die and a mothman would take your place. A generous GM might let you play as a mothman, but being an agent of fate seems like the kind of thing that has you fly off into NPC-dom.

Your stats would be replaced with that of the creature, like, you'd literally become one. Presumably you'd lose you class levels because this is a post-death effect and not a resurrection effect.

I'm sorry. :(

I think this is my fault.
