Overall, I'd recommend de-rhyming it and reformatting. I would also explore who "she" is more, and what about her makes her special to the narrator.

Edit: The last line is brilliant, by the way.

you had said goodbye.
But you see, goodbye means leaving, and leaving means forgetting.
I can feel you forgetting me.
You seem to forget that you, that i see now is the same person i love.
You forget that you're only human.
you don't have to get it together everyday
I want to know what its like to wake up and not feel sad.
I want to know you inside and out. because we're human, and to be human means to forget.
Just because we aren't together doesn't mean i don't love you Feelings never seem to make sense.
They just make you confused.
They drag you around for hours and days,
but you always end up in the same spot.

This hit me hard. I wrote a poem called " i remembered you" its like this, would you check it out?

I like how open you are emotionally here, and also the bareness of it, there is little eidetic language but it inspires imagery, and we are given glimpses into your life. I'd only say I think that some of the phrases here are a little I dunno, tried and true, "at first glance" "full of life" "achieve your dreams" not neccesarily a criticism, as I feel your ending question is a smart turn of phrase. Enjoyed this keep it up!

I love this, oh I love this so. That is utterly beautiful. 

She looked so young at first glance,when I looked again she was close to death and too old to dance,decades can turn to minutes if you try and live your life right,she told me I was embarrassing,I'm always miserable and she's always full of life,one day I'll be in the middle,one day I'll be in my prime,when are you too old to achieve your dreams?,when do you stop being too young to die?
