If I’m bringing medical/dental files or reports to my small claims court hearing (NYC) should I redact the Doctor’s name who had completed the report? Or is that critical to making my case?

Don't redact or modify the forms in any way. If you alter the papers in any way, that may make them unusable as evidence.
