Not sure why, but my character's dialogue, reloads, and footsteps are all silent, plus ISAAC and recordings. All gunfire (including my own), other character's dialogue, and most of the environment still can be heard. I reset my audio to default, and didn't see anything weird in those settings. Has anyone else encountered this? Any idea how to fix it?

Try google, resetting your game, resetting your console or pc, or like someone suggested uninstall and reinstall.
What platform? When this happened to me on Xbox it was a sound setting.
It is most probably an Atmos for Headphones glitch.
Try switching off any effects of your sound device. If there are no effects, you still need to switch them on and then off.
Usually, I just Alt+Tab during the game, right click on the sound icon in the Windows tray, switch Atmos on and then switch Atmos off. Helped every time!
It’s buggy. Delete and reinstall is your best bet.
Reinstalling would not fix an issueime this and there's zero reason to do so on a game so large.
Reset all audio settings to default as a start but unfortunately OP asked for help but gave zero information we could use to actually help them such as what platform they are on, headphones type, etc.
Eitherway, you can usually do the same thing that reinstalling accomplishes on most platforms without having to actually redownload 80 gbs again.
