I like it when poems have swear words (but not too much) which always makes me feel the narrator doesn’t take themselves too seriously and the list of WebMD catagories, the list of threes works well in parallel with “the dirty” “tbe diseased “ the skittish” 
The question at the end serves to take the poem once again out of traditional form, esp when it addresses the reader directly. The last line is only 3 syllables which leaves the reader with a powerful impact. The brevity sends its message. The only thing I would point out is I think you mean “categories” instead of categorize which is a verb, or you can say “categorizes - as”

1.Mental Illness 
2.Substance Abuse 
3.Trauma 
I've placed in each!

However, my breed, like a stray, is untrusting of humans.
We are "the dirty", "the diseased", "the skittish".

Do you know the number 1 reason a stray dog presents as "skittish" or "aggressive"?
Out of fear.
