Universal Facesheet Software for different EMRs? Does such a thing exist?
I work for a parent company that owns smaller companies with different disciplines (home health, hospice, rehab, palliative, etc). Each of these unfortunately use a different EMR. Has anyone had any luck creating a universal facesheet so when someone calls in, staff can pull up the same style facesheet for the patient regardless of which company/emr they're in?

The best place to look is probably in the SMART app gallery. Here’s one example: https://apps.smarthealthit.org/app/patientshare and another https://apps.smarthealthit.org/app/lush-smart-on-fhir-app from a quick search for something like what you’re asking for.
I think the challenge will be that the tools for non-physician practice disciplines are much less likely to support SMART and FHIR and so the standards-based mechanics that allow these apps to work won’t be supported by your home health or hospice or other tools.
Another alternative will be the pop health and care management tools. Think Lightbeam or ZeOmega Jiva (and Epic, Cerner and other EHR vendors have similar products). These are not cheap or easy solutions though and way overkill just for the ‘facesheet’ (which is based on a combined longitudinal patient record) as they are most commonly used to combined clinical data with claims (and SDoH too).
I don’t think there is an easy, affordable off-the-shelf answer for what you are asking for. I wonder if the time, energy and money to combine the data from the different systems to be able to render a shared facesheet might well be better spent on migrating to a vendor solution that can support all the disciplines directly. But maybe perhaps some of the info above can help with your search. Good luck!
This is beautiful. Thank you for such great input.
